NEW PATIENTS
Your Very First Visit
The first visit to our office is one of the most important visits – that’s why we reserve more time so we can focus on your interests and needs. These first visits allow us to address any special concerns that you may have, establish a baseline, and discover the current status of your oral health.
Your first exam and screening will usually include:
- Discussion of your needs, concerns, and desires
- Digital x-rays
- Periodontal exam and pocket measurements
- Oral cancer screening
To make sure you are being seen just as soon as possible at the time of your appointment, please remember to bring:
- Completed Health History Form
- Any insurance cards that might apply to your or your family
- Photo ID
- List of current medications
Insurance:
We will file your dental claims and accept most PPOs for the following insurance companies:
- Aetna
- Blue Cross Blue Shield
- Delta Dental
Every dental insurance plan has variations. Please contact our office and let us estimate your copays prior to your dental treatment.
Payment and Financing Options:
As payment for your dental appointment, we accept: cash, check, Visa, MasterCard, American Express, Discover and Flex Spending Cards.
